	(continued...) Reservations and Deposits Space is limited to 10 persons minimum 15 persons maximum. Reservations are accepted in order of receipt of a $500 deposit, a signed application for each participant, and a copy of your passport . Total payment is due 60 days prior to the start of the tour and should be transferred to Peru Viajes y Gourmet SAC in Peru. Please contact Penelope Alzamora for transfer instructions. Cancellation and Refunds I understand that full payment is due no later than 2 months in advance of the first day of the tour. Cancellation must be received in writing. Cancellations fee are based on per person and are as follows: :: 60 days before departure date – loss of deposit only. :: 31 to 59 days before departure date – 50% of total trip cost (including single supplement). :: Within 30 days of departure date – no refund. Responsibility Services provided in connection with the trip, such as hotel and all forms of transportation, are rendered by independent contractors who are not agents or employees of A Taste of Peru and Peru Viajes y Gourmet SAC. A Taste of Peru and Peru Viajes y Gourmet SAC arranges for these services only as agent for the individual participant. Participants expressly agree that A Taste of Peru and Peru Viajes y Gourmet SAC shall not be liable for any delay, mishap, inconvenience, expense, bodily injury or death or damage to property occasioned through the acts, negligence or defaults of any company or individual engaged in providing the services arranged by A Taste of Peru and Peru Viajes y Gourmet SAC. A Taste of Peru and Peru Viajes y Gourmet SAC shall not be liable, directly or indirectly, for any injury, death or damage for any reason whatsoever, including but not limited to acts of God, detention, war, quarantine, weather, terrorism, civil disturbance, strike, changes in government regulations, airline, or other travel operations. A Taste of Peru and Peru Viajes y Gourmet SAC will not be responsible for any belonging damaged, lost, or left behind by the participant. A Taste of Peru and Peru Viajes y Gourmet SAC reserves the right to substitute itinerary items of equal value without notice to participants. Signature on the reservation form of the participant or authorized representative signifies acceptance of and agreement to be bound by all the terms and conditions as stated in this brochure. Purchase of baggage, personal effects, accident, health, and trip cancellation insurance is strongly recommended. Participant expenses incurred in preparation for the trip (e.g., non-refundable purchased airline tickets) are not the responsibility of A Taste of Peru and Peru Viajes y Gourmet SAC. I enclose my deposit of $500 made payable to Penelope Alzamora, along with a copy of my passport, and acknowledged that the booking is made upon the above conditions, which I understand.
